Ketone bodies are not normally excreted with your urine.
There are many different reasons for ketone bodies being in the urine. When the body breaks down its own fat to gain energy, ketone bodies are created. These ketone bodies can get into the urine through fasting or with various conditions.
Ketone bodies in the urine can make it smell sweet.
